Medication Forms

All students who are prescribed medication during the school day, or who need over-the-counter medications, must have the proper forms on file in the health office (doctor's order and parental consent).  These forms need to be renewed annually.  No child may transport his/her own medication.  The medication must be brought to school in the original and properly labeled container by the parent or adult designate.  This includes both prescription and over-the-counter medications.

Medications that are not picked up by the last day of school will be discarded.

If your child has asthma, a life-threatening allergy, or seizure disorder, please complete the appropriate action plan found on the main Nurse Page.

If your child has diabetes, please forward a Diabetes Care Plan to the School Nurse, signed by the student's Endocrinologist.
